Q-Biz sells products and services for authorized vendors on the basis of commission arrangements. There ase 2 Steps before we can sell your products or service through Q-Biz. First, your company must become an authorized Q-Biz Vendor by completing this form and reaching terms of a business agreement with Q-Biz. Once you are an authorized vendor, you will be able to submit your product or service for sale through the Q-Biz system.

It is best to review What is the Q-Biz Program and a Sample Q-Biz Product/Service Fact Sheet and Order Form before filling out this form.

The purpose of this form is to assist us in determining vendor qualifications for our Q-Biz program, and terms of agreement for this service. Once submitted, the form will dynamically prepare a preliminary, non-binding agreement on the next screen. Please print the screen; and, review the agreement. If the terms are agreeable, and you have signed it, please fax it to us. If you have questions, please call Arthur Rosenfield at 212-692-0704.

When approved and signed by both parties, your company will receive vendor authorization and will be able to submit products for sale through the Q-Biz system. Q-Biz reserves the right to accept or reject any vendor solely on the basis of judgement. No claims or representations are made by Q-Biz, its agents or representatives, other than those included in a written agreement between the vendor and Q-Biz.
